Medical reports for temporary disability, risk during pregnancy and risk during breastfeeding, and maternity leaves

The online version of forms for sick and maternity leaves and the medical report for temporary disability, as well as their downloading, are regulated

 

Temporary disability, risk during pregnancy and risk during breastfeeding, and maternity are recognised by means of a series of reports issued by the physician of the public health system or corresponding mutual society. They form part of the healthcare for civil servants.

The medical report forms for temporary disability, risk during pregnancy and risk during breastfeeding, and maternity leaves are included in the annexes of Order PRE/1744/2010. These have been updated by the Resolution of 29 November 2010 of the General State Civil Servant Mutual Society.

Online version of the forms

The online version of forms for sick and maternity leaves and the medical report for temporary disability is regulated by Resolution of 30 September 2009 of the General State Civil Servants Mutual Society, published in the Official State Gazette of 13 October 2009.

According to this resolution, MUFACE is responsible for replacing the report forms for others that can be obtained via electronic, computer and telematic technology. In addition, the online version of the report forms are included in the framework of purposes of Act 11/2007, of 22 June, regarding citizens' electronic access to Public Services, thus simplifying the exercise of rights and the fulfilment of obligations via electronic means, as well as encouraging proximity to the citizen and administrative transparency.

 

In this regulatory context, MUFACE is initiating the process of abandoning the former system of printing report books, and it will be progressively replaced by their electronic version, being downloaded from the internet.

By virtue of the aforementioned, the following is resolved:

The online version of the report forms, to which the mutual society members will have access via the MUFACE website.

  1. a) Forms for illnesses, accidents, and risk during pregnancy and breastfeeding
    b) Specific medical report in situations of temporary disability
    c) Maternity leave form

 

The electronic format of the report forms will be fully valid to be used in procedures for recognising temporary disability, risk during pregnancy and risk during breastfeeding and for granting the corresponding leaves.

This resolution of 30 September 2009 applies retroactively to 11 August 2009, which is when the online version of report forms were available to the users.
